PRE-REGISTRATION FOR 2008 SEASON
Please pre-register using the form on our website.  Pre-registration will allow us to improve our final registration process and allow us to determine number of players interested in playing in the fall season and team and coaching requirements.  We will also notify you of the final registration times and dates.  All players must register again each year.

FINAL REGISTRATION FOR 2008 SEASON

When: 
- Saturday, April 26, 2008 - 9:00 AM - 1:00 PM

- Monday, April 28, 2008 - 5:30 PM - 8:00 PM

- Saturday, May 3, 2008 - 9:00 AM - 1:00 PM
- Monday,  May 5, 2008 - 5:30 PM - 8:00 PM

A birth certificate is required to complete registration.    Registration is closed.  See Late Registration Below

 NOTE: Players MUST also REGISTER IN PERSON on one of the assigned dates, with a parent or legal guardian, in order to complete the  registration process (even if they have completed the on-line pre-registration form).  All players that do not complete the registration on one of the the above assigned dates will  not be guaranteed a chance to play this season.  Anyone still wishing to register after the final registration date will be required to fill out a "Late Registration Form" and mail it to Fox Valley Pop Warner Football. Inc  P.O. Box 943, Appleton, WI 54912-0943.  These players will be put on a "wait-list" according to the date the form is received and will only be allowed to play if openings are available.  A late registration fee of $20.00 will be charged and the player is at greater risk of being placed on a team further from their  home and/or school.  Applicant's registration fee is refunded for players not accepted in the program.

Player Eligibility Requirements:
Open to all boys and girls from the Fox Valley Area that meet the following requirements:
  -Ages 8-14 (as of July 31 prior to the fall season)
  -Grades through 8th grade (during fall season.)
  -Weight must be within Age-Weight Schematic

* Division placement for players is determined by FVPWF Inc.  

Registration Requirements:
-Completed registration form
-Paid Registration fee $75

-Fundraising participation
-Certified copy of birth certificate (new players/first time registration)
-Player weigh-in

Late Registration:
Late Registration starts after May 5, 2008 for those players that did not register at Plamann on the scheduled dates.

  1. Download the late registration form.
  2. Fill out the form.  Sign & date the form.
  3. Include  Registration fee $75 + $20 late fee ($95 total)
  4. Include certified copy of birth certificate (new players/first time registration)

Note: Players names are added to the wait-list according to the date they are received at PO Box.
          All players mailing in forms will be contacted within 2 weeks.
          Your money will be refunded if we are unable to place the player on a team.

General Information:
-The league will have five age/weight divisions for players ..
-The program provides players helmets, shoulder pads, hip and knee pads.
-The program provides team coaches.
-The program provides practice and game uniforms.
-Players must provide their own mouth guards and rubber cleats.
